Registration
Required:
*Two proofs of residency:
1. Closing papers from your home purchase or a notarized copy of your lease.
2. Copy of utility bill, such as electric, water, or cable.
*Most recent report card
If you are coming from outside of Broward County you will also need the following:
* A copy of your child’s birth certificate
* A copy of his or her Standardized Test Scores
* A current physical, signed by the doctor with their I.D. number,
dated within the last 12 months
* Florida 680 Health Form which lists your child’s immunizations.
